Research on community care led by family doctor contracting in elderly patients with hypertension
家庭医生签约为主导的社区护理在老年高血压患者中的研究

摘要

目的 将家庭医生签约为主导的社区护理应用于老年高血压患者中，对其应用价值进行探究。方法 选取2024年1月-2025年8月在某街道社区卫生服务中心收治的92例老年高血压患者，随机分组，对照组46例行常规护理，观察组46例行家庭医生签约为主导的社区护理，对两组干预结果进行比较。结果 干预前，两组收缩压（SBP）、舒张压（DBP）水平比较，不具显著差异（P>0.05），干预后，观察组各血压水平更低（P<0.05）；干预前，两组ESCA、MMAS-8评分比较，不具显著差异（P>0.05），干预后，观察组各评分更高（P<0.05）；两组护理满意度比较，观察组95.65%更高（P<0.05）。结论 对老年高血压患者来说，实施家庭医生签约为主导的社区护理既能改善患者血压水平，还可提高自护能力及服药依从性，提高患者护理满意度，具有较高应用价值。

Abstract

Objective To explore the application value of community care led by family doctor contracting in elderly patients with hypertension.
Methods A total of 92 elderly patients with hypertension admitted to the community health service center from January 2024 to August 2025 were selected and randomly grouped. 46 patients in the control group received routine care, while 46 patients in the observation group received community care dominated by family doctor contract. The intervention results of the two groups were compared.
Results Before the intervention, there was no significant difference in the levels of DBP and SBP between the two groups (P>0.05). After the intervention, the blood pressure levels of each group in the observation group were lower (P<0.05). Before the intervention, there was no significant difference in the ESCA and MMAS-8 scores between the two groups (P>0.05). After the intervention, the scores of each group in the observation group were higher (P<0.05). The comparison of nursing satisfaction between the two groups showed that 95.65% was higher in the observation group (P<0.05).
Conclusion   For elderly patients with hypertension, implementing community care dominated by family doctor contracting can not only improve the patients' blood pressure levels, but also enhance their self-care ability and medication compliance, and increase patients' satisfaction with care. It has high application value.
